NOSE AESTHETICS (RHINOPLASTY)

Rhinoplasty is a surgical procedure performed to change the shape of the nose. Since our nose is located in the central part of our face, it is the organ whose curvatures and irregularities are most often noticeable. Problems with the nose are very different from person to person and are usually specific to the person and facial structure. Since each nose structure and facial structure is different, rhinoplasty (nose aesthetics) surgery should also be designed specifically for the person and the face. The deformity of the nose may be familial, post-traumatic or due to a previous operation.

For successful rhinoplasty; There is a need for three-dimensional thinking, to determine aesthetic and functional problems by making detailed nose and face analysis, and to plan in detail the surgical techniques required for the solution of all these.

It is important that your nose surgery is successful both functionally and aesthetically, and that you can breathe and smell comfortably through your nose after surgery. In the same surgery, turbinate (nasal flesh growth) and deviation (nasal bone curvature) should be removed.

In order to achieve aesthetic and functional results that are considered successful in nose surgery today, it is important that your doctor has extensive experience in the field of nose surgery, and that he or she can follow and apply up-to-date knowledge, surgical techniques and techniques in nose surgery. At the end of the procedure, the success of rhinoplasty is evaluated by the fact that the nose is able to obtain results according to both the facial characteristics of the person and his wishes, as well as the fact that the person has a comfortable breathing situation through the nose.

Patients who apply for rhinoplasty are first examined intranasal with endoscopes to determine whether there are problems with breathing. Afterwards, the nose structures of the patients are illustrated from different angles, simulations are made on these pictures to understand the expectations of the patient, and it is aimed to give the patient an idea about how his nose will be after the surgery.

Rhinoplasty is performed under general anesthesia and takes about 2-3 hours. Although the operation is performed with open or closed technique does not affect the duration of the operation, whether the patient has had surgery before, the structure of the tip of the nose, the size of the nose, the history of previous trauma, skin thickness, etc. The duration of the surgery may change due to factors.

In rhinoplasty surgery, the bone part of the nose must be cut to shape it. Various methods such as hand saw, chisel, rasp, micromotor, piezo ultrasonic cutter can be used to cut bones.

Postoperative bruising and swelling vary depending on the skin characteristics of the people and some personal factors rather than the bone cutting methods mentioned above.

In terms of pain after the operation, especially in the first 6 hours, strong painkillers are given, but then painkillers and antibiotic drugs suitable for the use of the patient are prescribed to the patient for use.

Postoperative 1. After the plaster change and the removal of silicone tampons in the nose during the week, the patient is 1. Month ,3. Month ,6. Month and 1 . Year Year checks are carried out.

Revision rhinoplasty

These are corrective surgeries applied to patients who have had rhinoplasty before. Since the septal cartilages in the nose of the patients are usually used in the first surgeries, the most important problem in these surgeries is the need for cartilage, which is necessary for the repair of the nose. If this cartilage need cannot be met from the cartilage in the nose in the nose, the cartilage need is met from the auricular (ear) cartilage or costal (rib) cartilage.

Minor revisions are performed under local or general anesthesia, and almost all major revisions are performed under general anesthesia. Minor revisions take 30-45 minutes, while major revisions can take 3-6 hours. The length of time is determined by the multiplicity and difficulty of deformities, the need for rib cartilage and ear cartilage. If the risk in terms of anesthesia is not specified, the result is always important, not the duration of the surgery

PROMINENT EAR AESTHETICS (Otoplasty)

The surgery performed against ear deformity, popularly known as prominent ear, is called otoplasty. Prominent ear, which is the most common among congenital or acquired disorders, is caused by the angle of the ear with the skull being higher than normal. This discomfort, which causes the ear to appear protruding, may also be accompanied by the absence of folds in the inner part of the ear.

The aim of the operation is to normalize the angle of the ear with the head and at the same time to recreate these folds. Prominent ear surgeries are very diverse. Each doctor applies a different technique. There are many techniques in the form of prominent ear surgery and open ear surgery. The operation, which is performed with general anesthesia in children and local anesthesia in adults in a hospital environment, takes about 1-1.5 hours. After the incision made behind the ear, permanent sutures are made to correct the folds and angle where the cartilages should be.

After the operation, the patient returns home on the same day. The edema and swelling that occur resolves within 2-3 weeks and it takes 2 months for the ear to take its final shape. Since the incisions are made from the back of the ear, the scar is not visible.

UPPER EYELID AESTHETICS (BLEPAROPLASTY)

Eyelid aesthetics, whose medical name is blepharoplasty, is the name of the surgical correction of symptoms such as sagging, bagging, wrinkle increases, drooping of the upper lids and eyebrows, bagging and lines on the lower lids, which usually occur due to aging (it may be at an early age due to genetics, trauma and infection).

Although the procedure takes about 30-60 minutes, it can be performed under local or general anesthesia.

With eyelid aesthetics, the excess on the lids, especially the skin, sometimes muscle and fat tissues are also removed. This is a permanent operation and its effect lasts for 10-15 years.

Redness and swelling occur in the first 3 days after eyelid aesthetics and disappear within 1 week. A good surgery and subsequent cold application during this period greatly prevent swelling, redness and bruises. 1 month after the operation, the scars of the surgery disappear and the image is in its best form. It is possible to return to daily routine work 3 days after eyelid aesthetic (blepharoplasty) surgery.

FACIAL AESTHETIC APPLICATIONS

BOTOX APPLICATIONS

Botox applications are a fast and easily effective treatment that can be applied to patients of all ages and without surgery and incision. Botox injections target the facial muscles that allow us to make our facial expressions, relaxing the muscles and minimizing the movement of these muscles to stop lines and wrinkles from forming.

Dynamic wrinkles are most commonly seen on the forehead, between the eyebrows, around the eyes and around the mouth. The wrinkles around the eyes, also called forehead and crow's feet, and the lines around the mouth give people an older expression, and the lines between the eyebrows give a more angry and harsh expression. With Botox application, it is possible to gain a calmer and younger expression by reducing the contraction of these muscles. Botox application will have an indirect effect by reducing the muscle movements that cause wrinkles rather than correcting them directly.

Also Some personal habits can have a direct effect on the expansion of your jaw muscles (masseter). Excessive chewing gum, chewing hard substances such as ice, or chewing hard meat can all lead to an enlarged masseter muscle. Grinding teeth at night due to stress, that is, bruxism, can also cause your jaw muscles to expand.

Masseter botox is an aesthetic procedure in which the lower part of the face, where the masseter muscle is located, is thinned.

If the masseter muscle is too large, it can cause discomfort not only aesthetically but also physically. For this reason, this procedure is also applied in some cases with the aim of providing treatment both formally and functionally.

Botox provides a new option as a non-surgical technique to reshape the face by thinning it. Masseter botox offers the opportunity to change the shape of the face by thinning the chin with a scalpel-free, painless, zero swelling and suture scars.

With masseter botox, much longer-lasting, aesthetic and symmetrical results are obtained over time.

NOSE FILLER

Nasal filling, generally known as non-surgical rhinoplasty in the community, involves the injection of skin filling materials.

Nasal filling, which is a very good option for patients who do not want to have surgery, is also very easy to perform touch-ups.

In cases where the patient is not completely satisfied with the result, the procedure is reassuring as it is reversible and the nose regains its original shape afterwards, as the filling may disappear within a few months.

The procedure consists of filling the nasal tissue, usually applying an anesthetic cream for 20-30 minutes, and then injecting it into key areas determined by the surgeon and the patient, thus providing aesthetic improvement. The injection usually takes 5-10 minutes and is performed in a clinical setting. After the nasal filler injection, normal life can be continued without interruption. It is one of the aesthetic operations with the highest patient satisfaction, as there is no swelling, bruising and no obligation to stay at home to heal.

LIP FILLER

Lips are one of the most important points on the face both aesthetically and functionally. Due to its dense vascular structure and mobility, specially developed hyaluronic acid fillers are used in terms of filling on the lips. Although it is most commonly performed for lip augmentation, lip shaping and clarification of lip contour, it is also used to correct lip asymmetries, remove fine barcode lines on the lips and reduce lip edge sagging.

Before applying the filler to the lips, creams with anesthetic (local anesthetic) effect are applied and left for 15-20 minutes. Afterwards, sometimes only the red area of the lip, and sometimes only the lip contour, where the lip and the skin meet, are injected with very fine-tipped needles. It's a quick and easy application that is completed in as little as 10 minutes.

Very rare, there may be minimal swelling and edema after the procedure, swelling and bruising do not occur in most patients because the consistency of the filler we use is very soft. Hyaluronic acid filler injection used in lip augmentation is not permanent, it is absorbed by the body over time. The duration of the filler is 6-12 months, depending on factors such as the age of the person, lip structure, elasticity, properties, quantity and quality of the injected filler.

UNDER-EYE LIGHT FILLER

Under-eye light filler injection is a treatment technique that allows skin tissues to reflect light better by increasing the level of hydration in depth, beautifying skin color and increasing its elasticity, hardness and smoothness.

With under-eye light filling, the code difference between the under-eye and the cheeks can be eliminated and the more tired and older appearance can be eliminated. The hollows formed under the eyes can be brought in line with the cheeks and an elevation can be achieved around the detention. In this way, the eye area, which reflects the light better, gets rid of dark circles and under-eye bags with the effect of hyaluronic acid, and provides a long-term cellular renewal thanks to polyvitamins and amino acids.

During the application, this special mixture is injected evenly with the help of a thin injection that will not cause any pain or ache under the eyes. After the application, minor redness may occur at the injection points. This redness goes away on its own within an average of 3-4 hours. During this period, the injection areas should not be rubbed. No cream or cosmetic product should be applied to the face. At the end of 4 hours, the skin usually returns to normal.

Mesotherapy (Youth Vaccine)

One of the most important causes of signs of aging on the skin is the loss of hyaluronic acid, which is naturally found in the body. Naturally, giving it back can restore the skin to its youth. With this application, which is called a youth vaccine or moisture vaccine, hyaluronic acid is given back to the skin and makes the skin look moist and bright thanks to its water-retaining effect.

Youth vaccine can be applied to the entire face, neck, low-neck and hands. With this technique, which restores moisture and elasticity to the skin, the aging process determined by genetic and environmental factors can be delayed. The youth vaccine, which can be used to remove wrinkles and sagging, eliminate the loosening of the skin and moisturize, also protects the skin.

After the application of local anesthetic cream, the youth vaccine is applied under the skin with very fine needles and performed in a period of 15-20 minutes. 4 sessions can be applied with an interval of 2 weeks. The most effective results occur in the 3rd month. In order to increase its permanence and effect, it is very useful to perform follow-up treatments for 1 session at intervals of 4-6 months.

PEDIATRIC ENT

TONSIL SURGERY (TONSILLECTOMY)

In some children, around the age of 4-5, tonsils start to become a source of infection rather than protecting them by growing too much due to frequent infections, and sometimes due to their large size, they prevent breathing and eating and cause respiratory arrest at night. In these and the following cases, the tonsils should be surgically removed.

  • Recurrence of tonsillitis more than 3-5 times a year in children and more than 2 times in adults
  • Sleep apnea
  • Tonsil abscess
  • Suspicion of malignant tonsil tumor
  • Excessive tonsil enlargement that causes difficulty in breathing and swallowing
  • Chronic tonsillitis that causes breath / bad breath and swelling of the lymph nodes,
  • Chronic tonsillitis seen in people with heart rheumatism, heart valve inflammation or kidney inflammation
  • Adenoid and tonsil enlargements that disrupt the jaw structure

  • The tonsils are removed from the mouth, which is opened with a special instrument under general anesthesia, by stripping the surrounding muscle and covering. Subsequently, bleeding control is performed and the surgery is terminated. Classical techniques such as scissors and scalpels can be used to remove the tonsils, as well as electrical burning of the tissue or similar technologies.

    ADENOID SURGERY

Adenoid is a lymphoid tissue located in the space behind the nose (Nasal) and known to take part in the body's defense system. In children, especially between the ages of 3 and 7, it can cause a number of problems such as the size of the adenoid, occlusive effects and susceptibility to infection.

As occlusive effects; nasal congestion and difficulty in breathing through the nose, related mouth breathing, mouth opening, jaw and face development problems, snoring, respiratory arrest during sleep, restlessness in sleep, intellectual development problems related to sleep obstruction syndrome can be counted. Recurrent infections are not limited to adenoiditis, but can be listed as frequent sinusitis attacks, recurrent fluid accumulation in the middle ear and middle ear suicide.

In such cases, adenoid surgery is recommended for therapeutic purposes. Adenoid surgery is a surgical procedure that takes about half an hour under general anesthesia. It is usually performed by an intraoral approach. Although various methods such as traditional cold surgery, electrocautery, coblation and laser are used, their effectiveness is similar. Patients who are kept under observation in the hospital for the first 4-6 hours after surgery and do not have any problems are discharged with antibiotics and painkillers. After discharge, a doctor's check-up is usually appropriate once a week later. Although rare, there may be bleeding, breathing difficulties and anesthesia-related problems after the surgery, in such cases, patients can be operated again.

    EAR TUBE SURGERY

Tubulation is also the process of placing small tubes in a hole in the eardrum in recurrent acute otitis media, middle ear fluids that do not heal on their own or with medical treatment, eardrum collapses, barotraumas that occur during flight or diving, patients where the eardrum or Eustachian tube does not develop correctly, such as down syndrome, cleft palate. Thanks to this procedure, fluid accumulation in the middle ear is prevented, both the child's hearing status is improved and the problems that may arise in the long term due to this fluid accumulation are prevented.

MThe fluid in the middle ear is pulled out by vacuum through the hole in the eardrum, which is drawn by advancing with microsurgical techniques. Then, a tube is placed with one end in the outer ear and the other end in the middle ear by means of special tools. After this procedure, there is no change in the person's ear that can be observed from the outside. In this operation, which is performed under general anesthesia, there is no need to make an incision in the skin.

KThe ear tube usually remains in the eardrum for 6-12 months; Then it spontaneously falls into the ear canal and the hole in the eardrum where the tube is attached closes on its own.
